Welcome to our Practice . . .

Our office is located in the Physicians Office Building on the west side, behind Trinity Regional Medical Center. You do not need to register downstairs, come straight to our office which is located on the second floor of the Physicians Office Building (West).

Please fill out your information sheet and health history form completely before arriving_for your appointment. Also, bring your current insurance cards to your appointment so we can get a copy of them to keep in your chart for references. We will not be able to file your insurance without the proper insurance information. Make sure you bring a list of your current medications that you are taking, and also a list of medication that you are allergic to.

Our office hours are 8:30 a.m. - 12:00 p.m. and 1:00 p.m. - 5:30 p.m. Monday through Friday. The office is closed on the weekends.

Our telephone number is 515-576-3100 or 1-800-360-4442. Routine appointments are scheduled during regular business hours. Our telephone is answered 24 hours a day, 7 days a week. If you should have an emergency after hours or wish to speak to the Doctor during that time, please phone our regular number and an operator will assist you with your call.

If you have any questions concerning your appointment, insurance questions, or anything about your statement, please call and speak to our clinic manager. She will be happy to assist you with any questions you may have.
